Clinical Study of Peptic Ulcer Complicated with Massive Bleeding

Abstract
78 cases of bleeding peptic ulcer treated at Dept. of general surgery, National Medical Center from May 1958 to April 1968 is reviewed.
1. Incidence of massive hemorrhagic peptic ulcer is higher in Male patient than that of Female.
2. Considering the seasonal variation, more frequent attacks in Spring and Autumn among four seasons.
3. Out of 78 patients, 60 had received operation.
Gastric resection was performed in 43 cases (71.7%) with its mortality rate 7.0%.
Considering the lower mortality rate compared with other surgical management, emergency gastric resection is seemed to be favorable one in state of poor economic condition.
4. Ratio between gastric and duodenal ulcer was 2.4:1.
Higher mortality rate was noted in gastric ulcer.
5. Average amount of blood transfusion was ranged from 2,000 to 4,000ml.
6. Higher mortality rate in more prolonged history is noted.
7. Three fourths of total cases showed severe anemia (Most of the cases had less than 8.0 G% Hemoglobin and average value 6.7g%)
8. Causes of deaths were bronchopneumonia, in 2 cases imbalance of electrolotes 1 case, hypovolemic shock in 1 case and mis-matched blood transfusion in 1 case.
